Our parish database categorizes family members into four roles:
- PRIMARY CONTACT – The main adult contact for parish communications. Only one person per household can be the Primary Contact. If unspecified, we will default to the first adult listed on the form.
- SPOUSE – The second adult in the household, if applicable.
- CHILD – A child of one of the adults in the family.
- OTHER – A household member who may not fit in the above categories, such as a partner of the Primary Contact.
This form allows you to register all individuals in your household under these roles.
